Have you ever been in this situation: the procedure says D1 is about "establishing a cross-functional team," so you send out a meeting invite for 2 PM. You walk to the meeting room, door's locked. You unlock it, boil some water, wait fifteen minutes. No one shows up. The quality manager's assistant texts "he's in a weekly review," the production supervisor says on WeChat "line's down can't come," the equipment engineer says "I didn't design the machine why are you asking me." So you sit down alone and start writing an 8D that was never meant to be written by one person. I've been in quality for fifteen years. The hardest part was never the technical stuff — material failures, parameter drifts, fracture mechanics, you can always figure those out if you give yourself enough time. What really drains me is that empty meeting room every time we kick off an 8D. You'd think D1 is the easiest step right? Just get a few people in a room. Wrong.
