SCAR Report — Supplier Corrective Action Request

A Supplier Corrective Action Report/Request (SCAR) is the document a customer sends to a supplier when a nonconformity is detected. The supplier responds, typically using the 8D format, with evidence of containment and correction.

What a Strong SCAR Response Includes

• D1 — team and roles
• D2 — specific problem description with 5W2H + evidence
• D3 — containment with quantities and dates
• D4 — root cause (5-Why) + escape point
• D5–D8 — corrective action, verification, prevention, closure
