Supplier 8D Report — Respond to a Customer Complaint
A supplier 8D is judged by the customer, not by the supplier. This guide shows what to put in each section so the response closes on the first submission.
The response sequence
D0–D1 — acknowledge the complaint, form the team and state the emergency response.
D2 — restate the defect exactly as the customer described it, with data and photos.
D3 — certify containment: inspected stock, clean point and certified date code.
D4 — prove the cause with tests; name the escape point in your own process.
D5–D7 — corrective action, verification data, and prevention in FMEA and control plan.
Why responses get rejected
Containment stated without a clean point or certified lot number.
Root cause phrased as "operator error" without a systemic fix.
No measured effectiveness data after implementation.
Prevention limited to the one line, with no horizontal deployment.
