FMEA & 8D Integration

I have walked into factories where the PFMEA had not been touched in three years despite having fifty closed 8D cases in the database. Let that sink in. Fifty problems investigated, fifty root causes identified, fifty corrective actions implemented and verified. And the risk assessment on the production line still looked exactly the same as it did before the very first incident occurred. If you close an 8D without updating the FMEA you have not prevented recurrence.
